The network was established in 2011 with the support of a $500,000 grant from The Duke Endowment and has now received more than $3 million from the organization.

“The continued partnership and support from The Duke Endowment provides crucial resources to Tidelands Community Care Network as we reach out to those in need in our community, helping individuals gain access to a broad range of health care services,” said Bruce Bailey, president and CEO of Tidelands Health. “Every day, we see the difference Tidelands Community Care Network makes in the lives of those we serve, and that wouldn’t be possible without the generous support of The Duke Endowment.”

The Duke Endowment’s support for Tidelands Community Care Network is part of a longstanding partnership with Tidelands Health over the past 25 years. In that period, The Duke Endowment has granted more than $4 million to the health system.

“We have enjoyed a long and productive relationship with Tidelands Health,” said Lin Hollowell, director of the endowment’s Health Care program area. “We are excited to continue our support for Tidelands Health’s ongoing efforts to improve community health and to expand access to care for the most vulnerable.”

Since its inception, Tidelands Community Care Network has served more than 6,000 people by connecting them with access to primary care, specialty care, affordable medication, behavioral health care, dental care, transportation and more. The network continues to grow and evolve to meet community needs.

One of the network’s greatest community benefits is its role as a facilitator to foster partnerships and increase cross-sector collaboration. The result is stronger links within the community, leading to integrated care and better health outcomes. Tidelands Community Care Network has more than 30 partners representing a range of government, non-profit and faith-based organizations.

“Bringing such a diverse group of agencies together provides tremendous benefit to the people we serve,” said Kelly Kaminski, senior director of community health resources at Tidelands Health. “It’s revolutionized the way we overcome the barriers many in our community face in accessing needed health care services such as transportation to medical appointments, availability of nutritious foods, suitable housing and more. Working together through Tidelands Community Care Network breaks down those barriers to help people live better lives through better health.”
